Entrance Exam Registration Form

All students wishing to be considered for admission into the 9th grade must take the entrance exam. First consideration will be given to students who take the entrance exam at Chaminade-Madonna. Students wishing to take the entrance exam at Chaminade-Madonna, must fill out the registration form below, then submit it online by clicking Submit Registration button @ bottom of this page. Bring the $ 50 exam fee on Saturday or call X 113 to pay by Credit Card.
